Ten days ago, the US Food and Drug Administration (FDA) announced that a sample of iceberg lettuce from Taylor Farms de Mexico tested positive for Cyclospora, the microscopic parasite responsible for a nine-state outbreak of foodborne illness.
The next day, the agency noted it was a false-positive test result.
Some people thought this meant that iceberg lettuce did not contribute to the thousands of cases of cyclosporiasis, the illness caused by Cyclospora. A day later, the FDA held a media briefing to explain that the false-positive was from lettuce that underwent regular screening at the border—not of the lettuce connected to the outbreak. Taylors Farms lettuce was still suspected, however.
By that time, many felt confused.
“This is a complicated outbreak. There’s no question about that,” Michael Osterholm, PhD, MPH, director of the University of Minnesota’s Center for Infectious Disease Research and Policy, which publishes CIDRAP News. “Even under the best of circumstances, this would not be an easy one to solve.”
Delays in onset, diagnosis slow investigation
Tracing Cyclospora remains difficult. Scientists can’t grow it in culture and use whole-genome sequencing to trace it. While the government is using partial genotyping to investigate the ongoing Cyclospora outbreaks, much of the information used to uncover the source comes from the people sickened by it. But how the parasite affects people also muddies the investigation.
“If you look at the timeline for this pathogen versus say E coli or Salmonella, it’s much more complicated,” Osterholm said.
“You’re talking about an incubation period [time from exposure to first symptoms] often of seven to 10 days. You’re talking about a period of illness that can be indefinite. But people don’t often go in and get tested until they realize ‘Maybe I have this particular parasite because I actually ate certain food products.’”
